Uniform Payment Plan Registration Form

After completing this form your athlete will be officially in good standing to receive their uniform. We will be taking all measurements on the first day of practice so we do not need them prior. Watch your email for your first installment invoice which will be sent within 3 days of filling out this form. It must be paid within 24hrs of receiving it or the agreement is void.
